Obesity in Scotland is one of the highest in Europe, with two-thirds of adults overweight, and one-third obese.
The consequences of obesity and inactive lifestyles include the risk of developing a range of long-term conditions (LTCs) including Type 2 diabetes, cardiovascular disease, and hypertension.
Obesity can affect not only our health but also our ability to lead happy, fulfilling lives.
Supported self-management models of care ensure that citizens play a key role in guiding their own care, in collaboration with Health Care Professionals and there is compelling evidence that supported self-management leads to better clinical outcomes, improves well-being and satisfaction with care received, and reduced health care utilisation.
The use of digital health technologies is a key enabler with real value being derived from co-designing and integrating technologies in a way that can be applied and adapted across many existing NHS service pathways to better support patients to self-monitor and sustain healthy lifestyles and behavioural changes.
DHI is working with industry partners (Site Kit) and Public Sector partners to innovate a digitally integrated solution to support weight management and active living using Moray Region as a test bed.
Designed around the use of a Personal Health & Care Record Application, we aim to develop and evaluate a digital solution that integrates remote clinical assessment, activity monitoring, dietary journaling, goal setting, and two-way messaging to support the delivery of a more personalised intervention approach for weight management, obesity prevention and promotion of an active lifestyle.
