DHI in collaboration with the Scottish Government Effective Prescribing and Therapeutics Division won the Patient Safety category award at the NHS Scotland Event
We are delighted to announce that DHI in collaboration with the Scottish Government Effective Prescribing and Therapeutics Division won the Patient Safety category award at the NHS Scotland Event for its poster Making the right decisions about medicines: national decision support for polypharmacy
Up to 11% of emergency hospital admissions result from harm from medicines, and 50% of these are preventable. This poster describes how patients and professionals have co-designed a suite of tools to improve safe, person-centred prescribing, using DHI’s national decision support platform – the Right Decision Service . The shared decision-making mobile app and patient-specific safety prompts in primary care electronic record systems are already delivering impact on safety and patient experience, with plans to expand and roll out more widely.
